How ChiropracticMatch handles your information

Information you provide

ChiropracticMatch collects the details you choose to submit through patient match requests and provider applications. Those details may include your name, contact information, ZIP code, accident timing, description of what you are feeling, communication preference, or practice and credential information. Directory claim and correction requests may also include a listing ID, NPI, practice contact details, website, requested corrections, and an authorization attestation.

Verified practice representatives and community contributors may submit listing photos. A submission includes the photographed listing, a private email address, a contributor display name, and a rights attestation. Pending originals are stored privately for manual review. Approved images are resized, stripped of embedded metadata, and copied to public image storage. Rejected originals are deleted, and abandoned pending uploads are deleted after 30 days.

How submitted information is used

We use submitted information to review requests, operate the matching service, respond to users, evaluate provider applications, maintain service records, prevent abuse, and troubleshoot delivery problems.

When information is shared

Patient-request details may be shared with participating offices when needed to respond to the requested matching service. Information may also be processed by vendors that provide hosting, database, email, analytics, or other operational services. ChiropracticMatch does not make clinical, scheduling, billing, or insurance decisions for a participating office.

Public directory records

The city directory uses public NPPES practice records and Census geographic data. Public-record listing details are not collected from patient match requests. Offices may submit supplemental information, but email verification and manual review are required before approved details or submitted photos can be published. The contributor's display name and approved photo may become public, but the verification email address remains private. Visitors may report a published photo, and ChiropracticMatch may retain the report and optional contact email while it reviews the concern.

Published practice websites may come from an approved provider claim or a manual review of public identity, address, and phone evidence. Website click events identify the listing and page context, but do not store the destination URL or information submitted in a patient or provider form.

Analytics and form answers

The site may collect general usage events such as page visits, form openings, and completed form steps. ChiropracticMatch does not intentionally send names, phone numbers, email addresses, ZIP codes, injury descriptions, or other form answers to its analytics service.

Directory website clicks and phone taps may be counted to evaluate usage and, for clearly labeled sponsored placements, prepare billing records. Fraud screening uses a one-way hashed fingerprint derived from request metadata. Raw IP addresses and form answers are not stored in directory click events, and the fingerprint is cleared after its short retention period.

Retention and security

Submitted records are retained for service operations, request follow-up, fraud prevention, and applicable recordkeeping needs. Access should be limited to people and service providers who need the information for those purposes. No internet transmission or storage system can be guaranteed completely secure.

Your choices

You can choose not to submit a request. If an office contacts you, you can tell that office or ChiropracticMatch that you no longer want contact about the request. You may also ask about correcting or deleting information, subject to operational and recordkeeping requirements.

Important limits

Do not use ChiropracticMatch for emergency care. The service is not a healthcare provider, insurer, law firm, or emergency service. If you may have a medical emergency, call 911 or seek immediate medical care.
